In general, we manually patch. We have a set of 'beta' servers that are patched automatically and, the theory is, if those survive, then we're free to manually patch the more critical servers. The beta list should be representative of the environment, an while it does contain test, dev and non-critical production servers, I'm not sure I'd call it representative. We've just recently (past 6 months) started vigorously rolling out Windows 2008 and I don't know that we have a Windows 2008 box in
